Three reference forms: Only references who can assess your work experience should be used. Please include your name and signature on each form before giving it to the designated references. Mail each form to the reference with a stamped envelope addressed to the Bonham ISD Human Resources Department. Each reference will then mail the completed information to this office. Reference forms can be downloaded by clicking on the link below.
High school diploma or GED certificate and transcript(s) of any college courses completed: Copies can be attached to this online application or mailed to the Bonham ISD Human Resources Department.
Completed Criminal History Record Check Form (Available for download in the section located at the bottom of this page)
Completed SSA-1945, Statement Concerning Your Employment in a Job Not Covered by Social Security (Available for download in the section located at the bottom of this page)
